We are deeply committed to every person’s safety and take steps to carefully assess and manage risks in all of our activities. However, we recognize that there are inherent risks associated with outdoor gardening and youth programs. Each participant (or participant’s parent or legal guardian) must submit this completed form to participate in any Fresh Food Pantry activities. Acknowledgement of Risk, Waiver of Liability, and Consent for Emergency Treatment
I understand and acknowledge that participation in outdoor gardening or youth activities involves certain inherent risks, including but not limited to: physical exertion, injury from tools or equipment, slips and falls, failure to follow instructions, exposure to communicable illness, and actions of third parties not under the control of Fresh Food Pantry staff or volunteers.
I certify that I (or my child) am in good physical health and able to participate in these activities. I voluntarily assume full responsibility for any risks, injuries, or damages known or unknown, which might occur as a result of participation.
In the event of a medical emergency, I authorize the adult staff or volunteers of Fresh Food Pantry to obtain emergency medical treatment for me or my child from qualified medical personnel. I understand that all reasonable efforts will be made to contact me or my emergency contacts. I agree to bear any costs incurred from such emergency treatment, including transportation.
I hereby release, indemnify, and hold harmless Fresh Food Pantry, its officers, directors, employees, volunteers, agents, affiliates, and Redeemer Church from any and all claims, liabilities, or causes of action, including attorney’s fees, arising out of or related to any injury, illness, or damages that may occur during participation in Fresh Food Pantry activities.
